That's an impressive fish for a Tenkara rod. I don't think we give the Tenkara rods a fair shake when it comes to the size of fish it can handle. I think it can take more punishment than we give it credit.

12' Ebisu and 13.5' Amago, 12' Iwana with a conversion handle, and an Ito.

That's a North Carolina rainbow that's been in the river for a few seasons. Healthy strong fish.

It's been my experience that the depth of the holes or runs makes a difference on how many large fish are netted. In fairly shallow water the Tenkara rod can subdue a fish larger than it's designed for; but if the trout can dive into deep water, something's going to give--usually the tippet breaks or the hook pulls out. I have heard of Tenkara fisherman having cord tied around the handle of the rod and then looped around a wrist. I haven't done this yet, but it might be a good idea in waters that harbour large trout. Fighting a large fish, feeling each plunge, jerk of the head, and enjoying the jumps are experiences best done with a Tenkara rod.

Is this a tailwater fishery? North Carolina isn't the kind of locale I'd expect to see a fish like that!

No, but these fish, benefit from supplemental feeding. Since NC streams are freestone and don't have the biomass that limestone streams have, fish are few and small without some sort of supplemental feeding. The only 'native' trout in NC are the brookies in the high country. Of course these fish average 4-5" in size. Many trout-fishing clubs have sprung up on NC rivers and streams, have improved the aquatic habitat, limited much of the poaching, and have increased the size and health of the fish.
